Accessing Your Medical Records – Subject Access Request (SAR)

In 2018 General Data Protection Regulations (GDPR) came into force and changed the patients rights to access to their health records. Patients can submit a Subject Access Request under GDPR for a printed copy of all or part of their record.

The Practice no longer has the right to charge a fee for providing the information.

In line with the GDPR, patients can submit a request for their medical record under a Subject Access Request (SAR). Any patient requesting a SAR must complete the following Application Form:

The completed form needs to be returned to the Practice with proof of identity.
